With the introduction of microsurgery, the field of plastic surgery has developed significantly as the indications for treatment of target diseases have expanded. In addition, microsurgery has made it possible to dramatically improve the therapeutic effect for diseases that could not be treated previously by conventional methods. Surgical treatment for lymphedema, such as lymphatic anastomosis and lymph node transplantation, is a topic of interest both in Japan and overseas, and our department is one of the few facilities in Japan that performs this treatment. In addition, for intractable ulcers such as diabetic gangrene of the foot and critical limb ischemia, microsurgery enables limb salvage even in cases where major amputation was unavoidable in the past with conventional methods, resulting in dramatic therapeutic effects. The role of reconstructive surgery after resection of malignant tumors is to improve the QOL of post-treatment patients both functionally and cosmetically. In breast reconstruction, we mainly perform reconstruction using autologous tissue, but also working on breast reconstruction while preventing lymphedema in the upper extremities secondary to lymph node dissection by adding lymph nodes. In the head and neck area, we not only restore the shape of the face but also perform functional reconstruction by composite tissue transfer. Microsurgery is also applied to limb injuries, and can be replanted by anastomosing the blood vessels of tissue that has been severed, such as an amputated finger. In this way, the scope of our work in reconstructive surgery is wide, and we would like to continue working on various advanced medical treatments in the field.
